STUDENT ENROLLMENT​ – PLEASE READ ALL DETAILS BELOW

For NEWLY ENROLLED AND EXISTING OAKDALE STUDENTS:  

  1. Go to the link below and enter each students’ name and birthdate into the EXISTING STUDENT UPDATE section.
  2. Complete each students permission forms.
  3. Upload your new residency forms (follow the required Residency Doc steps outlined below).
  4. Upload any new shot records. All in-coming 7th graders are required to have a current TDAP shot record and uploaded in the docs section or brought to the school.  Your student will not be able to attend school until proof is submitted and verified.

For NEW STUDENTS TO OAKDALE THAT HAVE NOT PRE-ENROLLED for 19-20 school year (This includes ALL Pre-K students): 

  1. Follow the link below and go the the New Student Enrollment section. 
  2. Complete ALL the prompts/forms to enroll your student.  This must be done for each student you are enrolling.
  3. Complete on-line Permission Forms. 
  4. Upload Residency Documents (see below for required steps), Shot Records, Birth Certificates and Home Language Survey.  Free/Reduced lunch forms are optional.

Other items to consider:

  • Enter (or estimate) the first day your student(s) will be attending school.  August 15th is the first day of school for the 19-20 school year.
  • All in-coming 7th graders are required to have a current TDAP shot recorded and uploaded in the docs section (or brought to the school).  They will not be able to attend school until proof is submitted.  
  • Read all the details and ensure you have the proper documentation in a correct file format (pdf, jpeg, png, docx).
  • Your enrollment is NOT complete until all the correct forms are submitted and verfified by school personnel. 
  • If you do not have the ability to upload documents, you must drop them by the school 1 week prior to your student attending in order for appropriate personnel to verify your enrollment.

RESIDENCY REQUIREMENTS

  • Students must live in the Oakdale School District, have legal documentation concerning guardianship, or have obtained a legal transfer which has been approved by the Oakdale Board of Education.
  • All residences are subject to verification by the Superintendent, the Principal, or his/her designee. (i.e. Registrar and/or Oakdale School Resource Officer.)
  • The OSSAA has a system to establish residency for the purposes of athletics. It is the district's intent to comply with those guidelines for the purpose of residency.

UPLOADING RESIDENCY DOCUMENTS

Required - Two documents (Proof 1 and Proof 2) must be used as proof of residence. Uploads for residency documents will only be available after 7/1/2019.  If you do not have the ability to upload documents, you must drop them by the school at least 1 week prior to your student attending in order for appropriate personnel to verify your enrollment.

One of the following must be uploaded as Proof 1:
  • Proof of payment of ad valorem taxes (property taxes) at an address located within the school district in which the applicant actually resides. To find this information use these links: OK County Assessor or OK County Treasurer.
  • Title or warranty deed to residential property in the district in which the enrolled applicant actually resides.
  • A valid unimpaired lease agreeement (showing valid dates, phone numbers, and addresses of both parties) in which the enrolled applicant actually resides.
  • If a family is living in a hotel/motel or RV park in the Oakdale district, they must provide a receipt payment from the current or previous month. Residency will be confirmed each semester.

One of the following must be uploaded as Proof 2:

  • A utility bill (water, gas, or electric bill only) at an address located within the school district in which the applicant actually resides. The bill must show a physical address that is located within the school district; no post office boxes will be accepted as proof of residence. Bills from July or August prior to the start of the school year are acceptable.
  • If a family is living in a hotel/motel or RV park in the Oakdale district, they must bring a signed letter from the manager confirming the family's stay. Residency will be confirmed each semester.

Residency Exceptions: Please contact Oakdale School to determine your residency if:

  • A family is living with a host family in the Oakdale School District, the host family must call to make an appointment to complete, sign, and notarize a residency affidavit attesting to another family's residence. All parties must attend. Residency will be verified at the beginning of each semester. [(The filing of a false affidavit shall be subject to punishment in accordance with 70 O.S. §1-113(A)(1)].
  • A family is currently building a home within the Oakdale School district.
